Credential Verification Fee

The American Board of General Dentistry is pleased to provide credential verifications for our members for various organizations, teaching institutions, and treatment facilities.

A handling fee of $20.00 per verification per individual is required. Payments can be made by check or credit card.

Checks and Charges must indicate name of requesting body (ie: name of hospital, university, etc).

Please send a request form and the handling fee to:

Verification requests with credit card payments may be processed by calling 561-809-5491.

No credential verifications will be given out over the phone or fax.
Credential Verifications will be processed within 72 hours of receipt.
